Position Description
The Integral Youth and Family Project, LLC



Position: Family Support Coordinator 
Reports To:
Executive Director, Deputy Director and Family Support Manager


Position Summary:
The Family Support Coordinator (FSC) provides professional support to families in pursuit of the goals of The Integral Youth and Family Project, Inc., and their own efforts to achieve greater levels of self- sufficiency and development. The incumbent assists families to become strong contributing members of any community in which they chose to live. The FSC counsels and coaches families, and through the use of various support and developmental programs, provides them with educational, recreational and employment opportunities.

This position is the second of a three level support staff structure, and is distinguished from the Family Support Tech in that the Family Support Coordinator is the main professional responsible for the delivery of direct support to families.


Job Responsibilities:

  1. Evaluates participant’s progress on an ongoing basis in their pursuit to achieve greater levels of self-sufficiency and development.
  2. Uses information and other creative means to locate and enroll families in a program of developmental activities tailored to individual needs.
  3. Assesses immediate needs and provides assistance and referrals for families and individuals.
  4. Determines the goals of each individual, and assists families in completing and maintaining the Family goals for the clients.
  5. Creates a program of activity for families/ individuals using in house or client preferred instruments and assists families/ individuals with the achievements of pre- determined measurable goals as established by the instruments.
  6. Retains close contact with families/ individuals while providing counseling and coaching for each person.
  7. Maintains individual confidential files containing records of the activities and accomplishments of each person.
  8. Attends community meetings.
  9. Prepares weekly reports for inclusion into the data bank of the project.
  10. Provides guidance to Family Support Techs assigned to their cases.
  11. Provides problem solving coaching and counseling techniques.
  12. Serves as a representative for the agency to network and collaborate with other
    community service providers.
  13. Performs crisis intervention and case management.
  14. Coordinates the delivery of services to clients as provided by this program.
  15. Works with the government and private management staff to provide seamless support for families.
  16. Participates in the training, working sessions and internal staff meetings to provide seamless support for families.
  17. Prepares reports for and attends meetings when required. These may include internal, external or other developmental team meetings.
  18. Provides excellent customer service to families, agencies, all staff, and other clients.
  19. Performs other duties as assigned by Executive Director, Deputy Directors, or Managers.


Knowledge, Skills, and Abilities required:

  1. A professional demeanor and strong leadership abilities.
  2. Ability to maintain confidential records on families in Case Management format.
  3. Ability to work with minimum supervision in the field and provide guidance for Family Support Techs.
  4. Ability to handle multiple tasks while consistently meeting deadlines.
  5. Ability to be creatively resourceful in the service of others.
  6. Ability to work successfully with diverse populations; bi-lingual a plus.
  7. Knowledge of various resources and services in Metro Atlanta and surrounding areas.
  8. Excellent verbal and written communication skills.
  9. Ability to work with the team, and community groups.
  10. Working knowledge of Microsoft Word, Excel, Access database and other related
    Software.
  11. Bachelor or Masters Degree in Social Work, Sociology Psychology, Human Resource Development and Community Development.


Work Environment:
Much of the working time is spent in the field. Incumbent must be self directed while maintaining connectivity to the team. Position requires significant amounts of travel in Atlanta and surrounding areas. Valid driver’s license and car required.
